3. Patient Responsibilities

Where applicable, telehealth or virtual consultations are subject to the following:

- Telehealth is not suitable for all conditions; an in-person visit may be required at the provider’s discretion
- Pratiksha Amin DMD uses HIPAA-compliant security measures, though no system is 100% secure
5. SMS Messaging Service

- We reserve the right to suspend non-emergency care for accounts with outstanding balances.
- No-show and late cancellation fees may apply as outlined in your No-Show Policy Agreement.

- Prescriptions are issued based on medical necessity and at the sole discretion of the treating provider.
- Controlled substances are subject to Texas state law and applicable federal regulations.
- Prescription refills may require a follow-up consultation before being issued.
8. Prohibited Conduct
Patients agree not to use our services for any of the following:
- Fraudulent, abusive, or illegal purposes
- Submitting false or misleading medical or insurance information
- Misuse or diversion of prescription medications
- Disruptive, threatening, or abusive behavior toward staff or other patients
- Unauthorized access to patient records or practice systems
Violations may result in immediate termination of services and referral for legal action.
9. Limitation of Liability
Pratiksha Amin DMD is not liable for service delays, website downtime, or limitations in telehealth technology beyond our reasonable control.
We do not guarantee specific dental or medical outcomes. Individual results may vary based on patient health, compliance, and other factors.
10. Modifications to Terms of Service
Pratiksha Amin DMD reserves the right to update these Terms at any time. Significant changes will be communicated via our website or in-office notices.
Continued use of our services following any update constitutes acceptance of the revised Terms and Conditions.
11. Governing Law & Dispute Resolution
- These Terms are governed by the laws of the State of Texas.
- Any disputes will first be pursued through mediation or arbitration before initiating litigation.
